Aims: The Thumb in opposition to the other fingers is the keypoint of the great tactile ability of the human hand. Literature postulates say, that movements of the thumb are transmitted from distally to proximally. This can explain the increased frequency of arthrotic changes in the radial carpal column. This experimental study for the first time examined the movement in the STT-(Scapho-Trapezio-Trapezoid)-Joint and the RS-(Radio-Scaphoid)-Joint during thumb circumduction. Methods: A magnetic-tracking-system (ISOTRACK) was used for data aquisition. 9 freshfrozen human cadaver hand specimen were used. The sensors were implanted in the first metacarpal, the trapezium and the scaphoid. Four experiments were performed: The kinematic behavior of the radial column during thumb circumduction with intact and dissected STT-ligaments combined with light and heavy muscle force. Results: There is a induction of movement through the radial column during thumb circumduction, the amount of which depends on hand position. There is a uniform transfer pattern in the radial column. Dissection of the STT-Ligaments increases carpal mobility. Conclusions: Extension and ulnar abduction of the wrist leads to a destabilization, flexion and radial abduction to a stabilization of the radial column. STT-Ligament injury causes a destabilization of the STT-Joint. This is suspect to be a main reason of develloping primary and secondary arthritic changes of the STT-Joint. For that reason the STT-ligament complex should be involved in conservative and invasive treatment of carpal pathologies. | |||||||
