Beschreibungen:Ziel der vorliegenden Studie war es, die diagnostische Aussagekraft der
immunzytochemischen Marker TTF-1 und Napsin A in der zytologischen Diagnostik
primärer Lungenkarzinome zu untersuchen und deren mögliche komplementäre
diagnostische Bedeutung zu evaluieren.
Hierzu wurden 251 zytologische Proben aus 238 Patienten aus dem Zeitraum Juli
2011 bis Januar 2013 retrospektiv analysiert. Das untersuchte Material bestand
überwiegend aus Ergusszytologien (116/251), gefolgt von Lungenproben (81/251)
sowie Lymphknotenpunktaten (44/251). In einzelnen Fällen stammte das Material
aus Leber-, Nebennieren- oder Pankreasläsionen.
Unter den untersuchten Fällen befanden sich 151 Adenokarzinome der Lunge. Der
Marker TTF-1 war in 113 dieser 151 Fälle (74,8%) positiv, während Napsin A in 108
Fällen (71,5%) exprimiert wurde. Bei den 162 nicht-plattenepithelialen
Lungenkarzinomen lag die Positivität für TTF-1 bei 71,0% (115/162) und für Napsin
A bei 68,5% (111/162).
Zwischen der Expression von TTF-1 und Napsin A bestand über alle 251 Fälle
hinweg eine hochsignifikante Korrelation: In 210 Fällen (83,7 %) waren beide Marker
übereinstimmend positiv oder negativ, in den übrigen 41 Fällen (16,3 %) reagierten
sie nicht gleichgerichtet.
Durch die kombinierte Anwendung beider Marker konnte die Sensitivität für den
Nachweis pulmonaler Adenokarzinome auf bis zu 86,1% gesteigert werden. Die
Ergebnisse zeigen, dass TTF-1 und Napsin A zuverlässige immunzytochemische
Marker in der zytologischen Diagnostik primärer Lungenkarzinome darstellen und
sich insbesondere in Kombination sinnvoll ergänzen.

The aim of this study was to evaluate the diagnostic performance of the
immunocytochemical markers TTF-1 and Napsin A in the cytological diagnosis of
primary lung carcinomas and to assess their potential complementary diagnostic
value.
A total of 251 cytological specimens from 238 patients, collected between July 2011
and January 2013, were retrospectively analyzed. The majority of samples consisted
of effusion cytology specimens (116/251), followed by lung samples (81/251) and
lymph node specimens (44/251). In a small number of cases, the material originated
from liver, adrenal gland, or pancreatic lesions.
Among the analyzed cases, 151 lung adenocarcinomas were identified. TTF-1
expression was observed in 113 of these 151 cases (74.8%), whereas Napsin A was
positive in 108 cases (71.5%). Among 162 non-squamous lung carcinomas, TTF-1
positivity was detected in 71.0% (115/162) and Napsin A positivity in 68.5%
(111/162).
The expression of TTF-1 and Napsin A showed a highly significant correlation across
all 251 cases: in 210 cases (83.7%), both markers were concordantly positive or
negative, whereas in the remaining 41 cases (16.3%) their results were discordant.
The combined use of both markers increased the sensitivity for the detection of
pulmonary adenocarcinomas to up to 86.1%. These findings demonstrate that TTF-1
and Napsin A are reliable immunocytochemical markers in the cytological diagnosis
of primary lung carcinomas, particularly when applied in combination.
