Beschreibungen:Das hochauflösende elektroanatomische Mapping des linken Vorhofs (LA) ist ein zentraler Bestandteil moderner Ablationsprozeduren bei Vorhofflimmern (VHF) und
linksatrialen Tachykardien (LAT). Es dient der anatomischen Rekonstruktion, der Katheternavigation, der Charakterisierung von Substrat und Aktivierung sowie der Überprüfung prozeduraler Endpunkte. Da Mapping und Re-Mapping in LA-Prozeduren wiederholt erforderlich sein können, gewinnt die Workflow-Effizienz der Signalakquise
zunehmend an Bedeutung. Ziel dieser Arbeit war es, den Einfluss der Mappingkathetergeneration auf die Workflow-Effizienz des hochauflösenden LAMappings zu untersuchen.
In diese retrospektive, monozentrische Beobachtungsstudie wurden 94 konsekutive Patienten eingeschlossen, bei denen zwischen Juli 2025 und Januar 2026 eine LAAblation
mit hochauflösendem Mapping durchgeführt wurde. Eingeschlossen wurden Prozeduren bei paroxysmalem VHF, persistierendem VHF und LAT. Verglichen wurden der Advisor HD Grid (Grid) und der Advisor HD Grid X (Grid X) in Kombination mit dem EnSite X 3D-Mappingsystem. Die Mappingzeit wurde als Zeitspanne vom Beginn der systematischen Signalakquise bis zur Erstellung einer vollständigen, klinisch verwertbaren LA-Map definiert. Zusätzlich wurden die finale Zahl erfasster Elektrogramme (EGMs), die EGM-Akquisitionsgeschwindigkeit und die
volumenbezogene Mapping-Geschwindigkeit bestimmt.
Bei weitgehend vergleichbaren Baseline-Charakteristika und Indikationsverteilung (Grid n=43; Grid X n=51) zeigte Grid X eine signifikant kürzere Mappingzeit (12 min [10-13] vs. 22 min [19-25], p<0,001) und eine höhere volumenbezogene Mapping-
Geschwindigkeit (6 ml/min [4,37-7,53] vs. 2,94 ml/min [2,14-4,9], p<0,001) als der Grid.
Auch die finale EGM-Zahl und die EGM-Akquisitionsgeschwindigkeit waren unter Grid X signifikant höher. Die explorative Subgruppenanalyse zeigte über paroxysmales VHF, persistierendes VHF und LAT hinweg eine gleichgerichtete Effektrichtung. Ein signifikanter Unterschied im akuten periprozeduralen Sicherheitsprofil bestand nicht.
Zusammenfassend war der Grid X im Vergleich zu Grid mit einer höheren Workflow-Effizienz des hochauflösenden LA-Mappings assoziiert. Die Ergebnisse sprechen dafür, dass technische Weiterentwicklungen innerhalb eines grid-basierten Mappingkonzepts die praktische Durchführung des LA-Mappings messbar beeinflussen können.

High-density electroanatomical mapping of the left atrium (LA) is an essential component of contemporary LA ablation procedures for atrial fibrillation (AF) and left atrial tachycardia (LAT). It supports anatomical reconstruction, catheter navigation,
substrate and activation assessment, and verification of procedural endpoints. Since mapping and remapping may be required repeatedly during LA procedures, workflow
efficiency of signal acquisition has become increasingly relevant. The aim of this dissertation was to evaluate the impact of mapping catheter generation on the workflow efficiency of high-density LA mapping.
This retrospective, single-center observational study included 94 consecutive patients who underwent LA ablation with high-density mapping between July 2025 and January 2026. Procedures were performed for paroxysmal AF, persistent AF, and LAT. Advisor HD Grid (Grid) and Advisor HD Grid X (Grid X) were compared, both used with the EnSite X 3D mapping system. Mapping time was defined as the interval from the start
of systematic signal acquisition to completion of a full, clinically usable LA map. In addition, the final number of acquired electrograms (EGMs), EGM acquisition speed, and volume-normalized mapping speed were assessed.
The study cohort consisted of 43 patients in the Grid group and 51 patients in the Grid X group. Baseline characteristics and procedural indications did not differ significantly between the two groups. Mapping time was significantly shorter with Grid X than with Grid (12 min [10-13] vs. 22 min [19-25], p<0.001). Volume-normalized mapping speed was significantly higher with the Grid X (6 ml/min [4.37-7.53] vs. 2.94 ml/min [2.14-4.9], p<0.001). The final number of acquired EGMs and EGM acquisition speed were also significantly higher in the Grid X group. Exploratory subgroup analysis showed a consistent direction of effect in favor of Grid X across paroxysmal AF, persistent AF, and LAT. No significant difference in acute periprocedural safety was observed.
In conclusion, Grid X was associated with higher workflow efficiency of high-density LA mapping compared with Grid. These findings indicate that technical refinements within a grid-based mapping concept can measurably affect the practical performance of LA mapping under routine clinical conditions.
