| Beschreibungen: | Während eines hämorrhagischen Schocks ist die intestinale Barrierefunktion entscheidend für die Entstehung möglicher Komplikationen wie einer Sepsis oder eines Multiorganversagens. Hier ist vor allem die mitochondriale Funktion und die regionale Mikrozirkulation im Gastrointestinaltrakt von Bedeutung. Remote ischemic preconditioning (RIPC) ist ein gewebeprotektives Verfahren, welches aus nicht letalen, kurzen Episoden aus Ischämie und Reperfusion an weiter entfernt liegenden Versorgungsgebieten besteht. Die Fragestellung dieser Arbeit ist die Beurteilung des Effektes von RIPC nach 24 h auf die intestinale mitochondriale Funktion unter physiologischen Kreislaufbedingungen und nach einem hämorrhagischen Schock mit anschließender Reperfusion. Zur Durchführung dieser Studie wurden 39 Wistar Ratten in 4 Versuchsgruppen unterteilt (Normovolämie (NV), Hypovolämie (HV), RIPC + NV, RIPC + HV). Zunächst wurden die Hinterläufe der Tiere mittels einer Blutdruckmanschette komprimiert (RIPC). 24 h später wurden die Tiere teils für 1 h einer Hämorrhagie mit anschließender Retransfusion ausgesetzt (HV). Die mitochondriale Funktion wurde mittels des Sauerstoffverbrauches in vitro anhand von Gewebehomogenaten des Dünndarms und des Colons gemessen. Glutamat/Malat und Succinat wurden hier als Substrate verwendet, um die basale Geschwindigkeit der Atmungskette (State 2) zu messen, während durch die Zugabe von ADP die Geschwindigkeit des maximalen Sauerstoffverbrauches (State 3) gemessen wurde. Als Maß für die Effizienz der oxidativen Phosphorylierung wurde die ADP/O-Ratio (zugegebenes ADP/O2 – Verbrauch) berechnet. Als Parameter für die Kopplung zwischen der Atmungskette und der oxidativen Phosphorylierung diente der respiratory control index (RCI = State 3/ State 2). Die Auswertung erfolgte mittels Kruskal-Wallis-Test und Dunns Korrektur (p≤0,05). RCI und die ADP/O – Ratio blieben nach Stimulation über beide Komplexe des Colons und des Ileums unverändert, sowohl im Vergleich zur Kontrollgruppe als auch nach RIPC. Für beide Komplexe zeigte sich hier ebenfalls kein signifikanter Unterschied sowohl im ADP/O - Ratio als auch im RCI nach Schockinduktion oder RIPC. Demzufolge konnte im späten Fenster der Protektion kein Einfluss von RIPC auf die mitochondriale Funktion in Dünndarm und Colon sowohl unter physiologischen Bedingungen als auch nach hämorrhagischem Schock mit anschließender Reperfusion nachgewiesen werden.During a hemorrhagic shock, the intestinal barrier function is crucial for the development of possible complications such as sepsis or multiple organ failure. Mitochondrial function and regional microcirculation in the gastrointestinal tract are of particular importance here.
Remote ischemic preconditioning (RIPC) is a tissue-protective procedure that consists of non-lethal, short episodes of ischemia and reperfusion in distant supply areas and has tissue-protective effects on organs. The purpose of this study is to evaluate the effect of RIPC after 24 h on intestinal mitochondrial function under physiological circulatory conditions and after hemorrhagic shock with subsequent reperfusion. To conduct this study, 39 Wistar rats were divided into 4 experimental groups (normovolemia (NV), hypovolemia (HV), RIPC + NV, RIPC + HV). First, the hind legs of the animals were compressed using a blood pressure cuff (RIPC). 24 h later, some of the animals were exposed to hemorrhage for 1 h (HV) with following reperfusion. Mitochondrial function was measured by oxygen consumption in vitro using tissue homogenates of the small intestine and colon. Glutamate/malate and succinate were used as substrates to measure the basal rate of the respiratory chain (state 2), while the rate of maximal oxygen consumption (state 3) was measured by the addition of ADP. The ADP/O-ratio (added ADP/ O2-consumption) was calculated as a measure of the efficiency of oxidative phosphorylation. The respiratory control index (RCI = state 3/state 2) served as a parameter for the coupling between the respiratory chain and oxidative phosphorylation. The evaluation was carried out using the Kruskal-Wallis test and Dunn's correction (p≤0.05). RCI and the ADP/O-ratio remained unchanged after stimulation across both complexes of the colon and ileum, both in comparison to the control group and after RIPC. For both complexes, there was also no significant difference in the ADP/O-ratio or in the RCI after shock induction or RIPC. Consequently, no influence of RIPC on mitochondrial function could be detected in the late window of protection, either under physiological conditions or after hemorrhagic shock followed by reperfusion. | |||||||
