| Beschreibungen: | Aufgrund einer hohen Metastasierungsrate und eingeschränkter kurativer Behandlungsoptionen hat das muskelinvasiv wachsende Urothelkarzinom (UC) eine schlechte Prognose. Die Relevanz epigenetischer Veränderungen in neoplastischen Geschehen hat in den letzten Jahren stark zugenommen. Dies gilt auch für die Pathogenese von Urothelzellkarzinomen, da sich im Vergleich zu anderen Tumorentitäten viele Mutationen bzw. aberrante Expressionen von epigenetischen Regulatoren finden ließen. Histon-Deacetylasen (HDAC) der Klasse I sind häufig überexprimiert im UC, was Veränderungen der Histonacetylierung nach sich zieht und sich global auf die Transkriptionsaktivität von Genen auswirkt. Daher sind solche HDACs ein beliebter Angriffspunkt in der Findung neuer Therapien mit HDAC-Inhibitoren (HDACi). Zu den HDACi der Klasse I gehört beispielsweise Mocetinostat. Erste klinische Studien mit dem HDACi Mocetinostat wurden jedoch aufgrund hoher Toxizität beendet. Eine weitere Möglichkeit, global die Gentranskription in Tumoren pharmakologisch zu remodellieren, sind selektive Inhibitoren von CDK9 (CDK9i), wie z.B. die Substanz LDC 000067. Diese stellen bereits in der Behandlung von anderen Malignitäten ein wichtiges Standbein dar. Im Rahmen der Entwicklung des neuen CDK9i MC 180295 konnte gleichzeitig eine epigenetische Wirksamkeit nachgewiesen werden, da CDK9 sich als wichtig für die Aufrechterhaltung der Heterochromatinstruktur erwies. Für das UC fehlen entsprechende Daten. Daher wurde zunächst in Urothelkarzinomzelllinien (UCC) die Häufigkeit von CDK9 überprüft. Anschließend erfolgte für die beiden UCCs UM-UC-3 und VM-CUB-1, sowie für die benigne uroepitheliale Kontrollzelllinie HBLAK, die Bestimmung der IC50-Werte für alle drei Inhibitoren (Mocetinostat, MC 180295, LDC 000067). Anschließend wurde untersucht, ob eine kombinierte Therapie aus HDACi und CDK9i in UCCs synergistisch wirkt, was ggf. eine Reduktion der Dosierung zur Folge hätte und damit zu einer möglichen Reduktion der Toxizität führen könnte. In der Untersuchung der Kurzzeitproliferation ergab sich für die Kombination aus Mocetinostat und MC 180295 (UM-UC-3) sowie für Mocetinostat und LDC 000067 (VM-CUB-1, HBLAK) ein synergistischer Effekt. Die Langzeitproliferation wurde vor allem durch den HDACi Mocetinostat negativ beeinflusst, jedoch war ein synergistischer Effekt im Rahmen der Kombinationstherapie nicht auszuschließen. Zudem konnte festgestellt werden, dass die Kombinationsbehandlung zu einer Zunahme der Zellzyklusdysregulation und zur Apoptoseinduktion führte. Mittels RNA-Sequenzierung nach Behandlung der UCCs und HBLAK mit jeweils einem der beiden CDK9i wurden differentiell exprimierte Gene bestimmt. Vor allem waren Gene betroffen, die an viralen Prozessen, an der Negativregulation antiapoptotischer Prozesse oder an immunologischen Prozessen beteiligt waren. Es konnte festgehalten werden, dass sich nach der kombinatorischen Behandlung mit dem HDACi Mocetinostat und einem der beiden CDK9i MC 180295 bzw. LDC 000067 ein leichter synergistischer Effekt erkennen ließ, dieser jedoch als nicht ausreichend für eine intensivere Untersuchung dieses Kombinationsansatzes gedeutet wurde.Due to high metastasis rate and limited curative treatment options, muscle-invasive urothelial carcinoma (UC) has a poorer prognosis. The relevance of epigenetic changes in neoplastic events has increased significantly in recent years. This also applies to the pathogenesis of urothelial cell carcinomas, as many mutations or aberrant expressions of epigenetic regulators have been found in comparison to other tumor entities. Class I histone deacetylases (HDACs) are frequently overexpressed in UC, which leads to changes in histone acetylation and has a global effect on the transcriptional activity of genes. Therefore, such HDACs are a favorite target in the discovery of new therapies with HDAC inhibitors (HDACi). Class I HDACis include mocetinostat, for example. However, initial clinical trials with mocetinostat were terminated due to its high toxicity. Another option for the global pharmacological remodelling of gene transcription in tumours are selective inhibitors of CDK9 (CDK9i), such as the substance LDC 000067. These inhibitors already represent an important pillar in the treatment of other malignancies. Epigenetic efficacy was also demonstrated during the development of the new CDK9i MC 180295, as CDK9 was found to be important for maintaining heterochromatin structure. However, there is a lack of corresponding data for UC. Therefore, the abundance of CDK9 was first tested in UC cell lines (UCCs). The IC50 values for all three inhibitors (mocetinostat, MC 180295 and LDC 000067) were then determined for two UCCs (UM-UC-3 and VM-UCB-1) and the benign uroepithelial control cell line HBLAK. Subsequently, the potential for a combined HDACi and CDK9i therapy to have a synergistic effect in UCCs was investigated, which could result in a reduced dosage and thus potentially reduce toxicity. The study of short-term proliferation revealed a synergistic effect for mocetinostat and MC 180295 (UM-UC-3), as well as for mocetinostat and LDC 000067 (VM-CUB-1, HBLAK). Long-term proliferation was primarily influenced negatively by mocetinostat (HDACi), but a synergistic effect in the context of combination therapy could not be ruled out. Furthermore, it was found that the combination treatment induced an increase in cell cycle dysregulation and apoptosis. Differentially expressed genes were determined by RNA sequencing following the treatment of UCCs and HBLAK with one of two CDK9is. Genes involved in viral processes, the negative regulation of anti-apoptotic processes, and immunological processes were particularly affected. Following combinatorial treatment with the HDACi mocetinostat and one of the two CDK9i (MC 180295 or LDC 000067), a slight synergistic effect was observed. However, this was deemed insufficient for further investigation of this combination approach. | |||||||
