| Beschreibungen: | Ziel dieser Arbeit ist die Validierung der Klassifikationssysteme für myelodysplastische Syndrome (MDS) anhand von Daten von 5700 Patienten aus dem Düsseldorfer MDS-Register.
In die Analyse wurden die Klassifikationen der WHO von 2022 sowie die ICC einbezogen. Zusätzlich flossen auch die WHO-Klassifikationen der Jahre 2001, 2008 und 2016 sowie das FAB-System von 1982 in die Untersuchung mit ein. Das Hauptziel dieser Einteilungen besteht darin, die Krankheitsgruppen der myelodysplastischen Syndrome möglichst präzise und homogen zu definieren sowie eine verbesserte Prognoseeinschätzung zu ermöglichen. Die Analysen haben gezeigt, dass sowohl die ICC- als auch die WHO-2022-Klassifikation auf Grundlage der vorliegenden Daten als prognostisch hilfreich validiert werden konnten. Insbesondere erweist sich die neue WHO-2022-Klassifikation, die morphologisch und genetisch definierte MDS-Gruppen aktualisiert, als äußerst homogen bezüglich ihres Überlebens, dieses spiegelt sich teilweise auch in der ICC-Klassifikation wider. Für die Klassifikationen wurden nicht nur der medulläre und periphere Blastenanteil, das Ausmaß der Dysplasiezeichen, der Anteil an Ringsideroblasten und Auerstäbchen sowie chromosomale (del5q) und molekulare (SF3B1, TP53) Veränderungen berücksichtigt, sondern es war auch eine histologische Beurteilung der Zellularität und Fibrose in der WHO-2022-Gruppe von großer Bedeutung. Neben der bereits 2016 definierten molekularen SF3B1-Gruppe konnte sowohl in der WHO-2022-Klassifikation als auch in der ICC eine neue Gruppe hinzugefügt werden, die sich mit der molekularen TP53-Alteration beschäftigt. Die fortschreitende Detaillierung über die Jahre ermöglicht eine präzisere Beschreibung und genauere Zuordnung der Patienten zu den einzelnen Gruppen. Dies führt zu einer besseren Prognoseeinschätzung und einem frühzeitigen Erkennen von Genotyp-Phänotyp-Korrelationen. Die Weiterentwicklung der Klassifikationen und die Einführung neuer Einteilungskriterien könnten in Zukunft auch Vorteile für die Behandlung von MDS-Patienten mit sich bringen.The aim of this study is to validate the classification systems for myelodysplastic syndromes (MDS) using data from 5,700 patients from the Düsseldorf MDS registry. The classifications examined include the WHO classification from 2022 as well as the ICC, along with the WHO classifications from 2001, 2008, and 2016, and the FAB system from 1982. The main objective of these classifications is to define the disease groups of myelodysplastic syndromes as precisely and homogeneously as possible, as well as to enable improved prognostic assessments. The analyses have shown that both the ICC and the WHO-2022 classification can be validated as prognostically helpful based on the available data. In particular, the new WHO-2022 classification, which updates morphologically and genetically defined MDS groups, proves to be extremely homogeneous in terms of survival, which is partially reflected in the ICC classification as well. For the classifications, not only the medullary and peripheral blast percentages, the extent of dysplastic features, the proportion of ringed sideroblasts and Auer rods, and chromosomal (del5q) and molecular (SF3B1, TP53) changes were considered, but a histological assessment of cellularity was also of great importance in the WHO-2022 group. In addition to the already defined molecular SF3B1 group from 2016, a new group focusing on the molecular TP53 alteration was added in both the WHO-2022 classification and the ICC. The ongoing detailing over the years allows for a more precise description and more accurate assignment of patients to the individual groups. This leads to better prognostic assessments and earlier recognition of genotype-phenotype correlations. The further development of classifications and the introduction of new criteria could also bring benefits for the treatment of MDS patients in the future. | |||||||
