Beschreibungen: | Als Vinculum breve des Daumens bezeichnet man eine Umschlagsfalte des Stratum
synoviale der Sehnenscheide des Musculus flexor pollicis longus auf Höhe des distalen Drittels der Phalanx proximalis [4, 14]. Es ist mit der palmaren Platte des Interphalangealgelenkes verbunden und führt Blutgefäße und Nerven zum Hilum der Sehne [4, 5, 7, 48, 58, 66, 72, 106]. Es folgt den Bewegungen der Sehne und stellt ihre Nährstoffversorgung auch bei hohen Gleitamplituden sicher [22]. Ruffini-Körperchen gehören zu den Mechanosensoren [88, 96]. Es handelt sich um langsam adaptierende Dehnungsrezeptoren der tieferen Körperschichten [12, 67]. Sie tragen unter anderem zur Empfindung der propriozeptiven Qualitäten Lage-, Stellungs-, Bewegungs- und Kraftsinn bei [88, 96]. Ein Zusammenhang zwischen dem Vinculum breve und der Registrierung propriozeptiver Qualitäten ist in der Literatur bislang nicht beschrieben. Im Rahmen dieser Arbeit wurden die Mechanosensoren des Daumens histologisch kartografiert. Ziel war die Darstellung bisher unbekannter Verteilungsmuster der Mechanosensoren des menschlichen Daumens. Um die Mechanosensoren zur kartografieren wurden histologische Übersichtspräparate fünf zuvor ausgewählter Höhen des Daumens erstellt. Dafür wurden Querschnitte auf Höhe des Carpometacarpal-, Metacarpophalangeal- und Interphalangealgelenkes sowie auf mittlerer Höhe des Os metacarpale I und der Phalanx proximalis an 15 Daumen von 8 formaldehydfixierten Körperspenden präpariert und mit einer Elastica-Hämatoxylin-van- Gieson-Färbung angefärbt. In 13 der 15 Übersichtspräparate auf Höhe des Interphalangealgelenkes konnten auf diese Weise Ruffini-Körperchen innerhalb des stets angeschnittenen Vinculum breve dargestellt werden. Zur Validierung dieses Ergebnisses wurden daraufhin weitere 16 Daumen von 9 Körperspenden präpariert und das Vinculum breve zur histologischen Untersuchung gezielt entnommen. Unter Elastica-Hämatoxylin-van-Gieson-Färbung zeigten sich hier in 15 von 16 Vincula breve ebenfalls Ruffini-Körperchen. Als Gesamtergebnis konnten somit in 28 von 31 Vincula breve Ruffini-Körperchen lichtmikroskopisch dargestellt werden. Im Kontext der Literatur lassen diese Ergebnisse vermuten, dass die dehnungssensitiven Ruffini-Körperchen Auslenkungen des Vinculum breve bei Bewegung des Daumens unmittelbar registrieren können. Diese Arbeit stellt jedoch eine rein morphologische Erfassung der Ruffini-Körperchen dar. Der Beweis, dass sie bei Auslenkungen des Vinculum breve tatsächlich aktiviert werden, könnte anhand einer physiologischen Studie weiter untersucht werden. Die lichtmikroskopische Darstellung von Ruffini-Körperchen im Vinculum breve des Daumens legt eine Beteiligung des Vinculum breve an der Entstehung propriozeptiver Empfindungen des Daumens nahe. Diese Beobachtung erhöht die Relevanz des Vinculum breve für die reguläre Funktion des Daumens und kann einen neuen Ansatzpunkt sowohl in der operativen Therapie als auch in der Nachbehandlung von Verletzungen oder Krankheiten des Daumens bieten.The vinculum breve of the thumb is characterized as an evagination of the synovial stratum of the flexor pollicis longus tendon and located on the distal section of the proximal phalanx [4, 14]. It is attached to the volar plate of the interphalangeal joint and carries blood vessels as well as peripheral nerves towards the hilum of the tendon [4, 5, 7, 48, 58, 66, 72, 106]. Thus, it can follow the tendon‘s motion and ensure its nutrition despite high amplitudes of excursion [22]. Ruffini-corpuscles are counted among the mechanosensory neurons [88, 96]. They are described as slowly adapting strain sensors of the deeper tissue layers and provide proprioceptive information such as the sensation of movement or position [12, 67, 88, 96]. The current literature provides no information on a connection between the vinculum breve and the perception of proprioceptive sensations. In the scope of this work, mechanosensory neurons have been mapped histologically. In addition, the study served to display so far unknown distribution patterns of the mechanosensory neurons within the human thumb. In order to map the mechanosensory neurons, large histological specimens providing a complete panoramic view of five previously determined levels of the thumb were established. Therefore, cross sections at the height of the carpometacarpal-, metacarpophalangealand interphalangeal joint as well as at medium height of the first metacarpal bone and the phalanx proximalis of 15 thumbs taken from a total of 8 formaldehyd-fixed cadavers were prepared and dyed with an Elastica-Hämatoxylin-van-Gieson-dye. 13 of 15 overviewspecimens at the height of the interphalangeal joint showed Ruffini-corpuscles within the vinculum breve. An additional 16 thumbs taken from 9 formaldehyd-fixed cadavers were dissected to gather targeted specimens of the vinculum breve to validate these findings. Dyed with Elastica- Hämatoxylin-van-Gieson-dye Ruffini-corpuscles could be displayed in 15 of those 16 vincula breve. In summary, 28 of all 31 vincula breve examined contained Ruffini-corpuscles. With regard to current literature, these results suggest that stretch-sensitive Ruffinicorpuscles may sense deflection of the vinculum breve during movement of the thumb. However, this study represents a mere morphological display of Ruffini-corpuscles. To prove their activation during deflection of the vinculum breve, an additional physiological examination would be required. The histological display of Ruffini-corpuscles within the vinculum breve of the thumb leads to assuming that the vinculum breve takes part in the perception of proprioceptive sensations of the thumb. These findings increase the significance of the vinculum breve for normal movement of the thumb and may serve as a new approach for surgical as well as follow-up treatment of traumatic injury or other disorders of the thumb. | |||||||
