Beschreibung:Chemical exchange saturation transfer (CEST) is a novel magnetic resonance imaging technique, in which low concentrated biomolecules can be detected indirectly through their magnetization transfer with water. The underlying proton exchange mechanism is affected by the labile proton concentration and exchange rate, which, in turn, depends on pH and temperature. CEST MRI offers, therefore, not only the possibility to detect certain cellular compounds, but can also provide insights into diseases on a molecular level.

This thesis aimed at exploring the potential of CEST MRI for functional kidney imaging. For this purpose, several most abundant kidney metabolites were investigated for their suitability in clinical CEST imaging. A particular focus was placed on urea, which is an important diagnostic marker of renal function. Additionally, different CEST acquisition protocols and quantification methods were evaluated using both simulated and phantom data. Finally, in order to enable the in vivo translation of CEST imaging to human kidney, an optimized CEST pulse sequence with a multi-echo readout and a post-processing protocol were developed, and subsequently applied in a clinical cohort of renal transplant recipients.
