Beschreibungen:Die Analyse der lokalen Immunantwort im Sulkus gingivalis gibt Auskunft über Immunparameter, unter anderem IL-1β, die für die Ätiopathogenese der Parodontitis von Bedeutung sind. Studien mit Analysen der Konzentration von IL-1β haben in der parodontologischen Forschung zum besseren Verständnis der Pathogenese der Parodontitiden beigetragen. Während viele Studien die Bedeutung von IL-1β und seine Funktion als proinflammtorischem Zytokin in der Parodontologie beschreiben, fehlen Studien, die untersuchen, ob die Konzentrationen von IL-1β einer zirkadianen Rhythmik unterliegen. Diese Informationen sind für Konzeption, Planung und Finanzierung zukünftiger Studien zu IL-1b in der Parodontologie von großem Interesse.
Wenige Studien zu IL-1β in ex vivo stimulierten Vollblutproben weisen darauf hin, dass die Freisetzung im Allgemeinen und damit auch möglicherweise im Sulkus gingivalis einer zirkadianen Rhythmik unterliegt.
Mit der vorliegenden Arbeit sollte daher überprüft werden, ob sulkuläres Interleukin-1β einer zirkadianen Rhythmik unterliegt.
Die zirkadianen Variationen, die zwischen 8.00 morgens und 22.00 Uhr abends auftreten, sind dabei von besonderem forschungspraktischem Interesse.
Außerdem sollte berücksichtigt werden, dass sich zirkadiane Rhythmen möglicherweise in Abhängigkeit von der klinischen Ausgangssituation unterscheiden, daher werden im Split-mouth Design zirkadiane Rhythmen in Quadranten mit und ohne Plaqueakkumulation verglichen.
Insgesamt 28 gesunde Probanden, davon 12 weibliche und 16 männliche im Alter von 22 bis 47 Jahren wurden zu Beginn der Studie untersucht. Nach der Eingangsuntersuchung erfolgte eine professionelle Zahnreinigung, um für alle Probanden einen plaque- und entzündungsfreien Zustand zu erhalten und gleiche Ausgangsbedingungen zu schaffen. Nach dem Zufallsprinzip wurden die Quadranten mit Plaqueakkumulation bestimmt, die Probanden wurden angewiesen, die Mundhygiene in diesem Quadranten zu vernachlässigen. Mit den gewonnenen Daten sollten Erkenntnisse über die zirkadiane Rhythmik von IL-1β im Sulkus Fluid bei guter Mundhygiene als auch mit leichter Plaqueakkumulation gesammelt werden.
In Hinblick auf Studien über Konzentrationsveränderungen von IL-1β in Anwesenheit von mikrobieller Plaque wurde vermutet, dass in Quadranten mit Plaque im Vergleich zu Quadranten ohne Plaque höhere IL-1β ­Konzentrationen im Sulkus gingivalis vorliegen. Es stellte sich allerdings die Frage, ob sich zirkadiane Rhythmen von sulkulärem IL-1β in Quadranten mit guter Mundhygiene von denen mit Plaque unterscheiden. Tatsächlich zeigte sich, dass sulkuläres IL-1β einer zirkadianen Variation mit Anstieg in den Abendstunden unterliegt. Außerdem ergaben sich tendenziell signifikante Interaktionen bei den Parametern Hygiene x Zeit x Geschlecht und Hygiene x Geschlecht.
Damit liefert die Studie Hinweise dafür, dass IL-1β im Sulkus Fluid einer zirkadianen Rhythmik unterliegt.

Background:
Interleukin-1ß is an important parameter in periodontal research because of its role in inflammation and bone resorption. One measure to assess local IL-1ß concentrations is its analysis in gingival crevicular fluid (GCF). While studies on serum IL-1ß concentrations indicate a circadian rhythm of this parameter nothing is known about daytime variations of IL-1ß in gingival crevicular fluid.
Aim:
To analyse daytime variations of IL-1ß in GCF.
Method:
Daytime variations of GCF-IL-1ß between 0800 and 2200 were assessed with a time resolution of 2 hours in 28 periodontally healthy subjects.
Results:
Data show a significant variation with daytime with lowest concentrations in the morning and highest in the evening. Effect sizes of comparisons between morning and evening samples are medium to high and correspond to those observed between healthy and periodontitis sites. Least daytime variations are found between 1200 and 1800 hours.
Conclusion:
Daytime variations in GCF-IL-1ß are large enough to be able to mimic or mask differences due to clinical factors. Thus daytime must be controlled in GCF-IL-1ß research, otherwise results become equivocal. Future studies are needed to assess daytime variations in other GCF-parameters and in diseased subjects.
