Beschreibungen:Einleitung: Neuropsychiatrische Symptome und kognitve Störungen sind mentale Manifestationen bei primären Hyperparathyreoidismus (pHPT). Das Ziel dieser Studie war es zu untersuchen ob eine operative Therapie zu einer anhaltenden Verbesserung dieser Symptome führt.
Methoden: In einer prospektive fallkontrollierten Studie wurden 30 Patienten mit einem pHPT und 30 Patienten mit einer euthyreoten Knotenstruma auf psychische und kognitive Störungen präoperativ und 6 Monatie postoperativ untersucht. Hierzu wurde die Hamiltondepressionsskala und vier Tests zur Beurteilung kognitiver Störungen verwendet: DemTect, MWT (mehrfach Wortschatztest), ZVT, Benton Test.
Ergebnisse: Patienten mit einem pHPT zeigten singifikant mehr kognive Störungen (p<0.0001) mit einer signifikanten Verbesserung 6 Monate postoperativ (p<0.0001). Patienten mit einem pHPT zeigten ebenfalls mehr psychopathologische Symptome als Patienten der Kontrollgruppe (p=n.s.) und es ließ sich eine tendentielle Verbesserung dieser Symptome postoperativ nachweisen. Es konnte keine Korrelation der Erhöhung des Serumkalzium- oder de Parathormonwertes mit der klinischen Symptomatik nachgewiesen werden.
Schlussfolgerung: Patienten mit einem pHPT weisen häufig neuropsychiatrische Symptome und kognitive Störungen auf. Eine erfolgreiche Nebenschilddrüsenadenomextirpation führt zu einer signifikanten Verbesserung der kognitiven Störungen.

Introduction: Neuropsychiatric symptoms and cognitive impairment are mental manifestations of primary hyperparathyroidism(pHPT). The aim of our study was to determine if surgical treatment results in a long-lasting full recovery from these symptoms.
Methods: In a prospective case-control study with machting pairs, mental changes were examined preoperatively and 6 month postoperaively in 30 patients with pHPT and 30 patients with nontoxic nodular goiter using the Hamilton depression score and four cognitve tests: DemTect, MWT (multiple word test) ZVT, and Benton test.
Results: Patients with pHPT demonstrated significantly more cognitive changes (P<0.0001) with significant improvement 6 month postoperativley (P<0.0001). Patients with pHPT presented more psychopathologic symptoms than patients of the control group (P=NS), and there was a tendency towards recovery in the pHPT group postoperatively. There was no correlation between biochemistry and psychopathologic or cognitive changes in the pHPT group.
Conclutions: Patients with pHPT often present with neuropsychiatric symptoms and cognitive impairment. A successful parathyroid operation improves cognitive disorders in particular.
