Beschreibungen:Die Ätiopathogenese entzündlich rheumatischer Systemerkrankungen wie der rheumatoiden Arthritis (RA), dem systemischen Lupus erythematodes (SLE) oder den Vaskulitiden ist bislang noch nicht vollständig geklärt. Es wird ein multifaktorielles Geschehen angenommen. Eine Hypothese geht davon aus, dass es bereits im Säuglingsalter zu Fehlentwicklungen des Immunsystems kommt. Zu den ersten exogen zugeführten Proteinen und damit potentiellen Antigenen zählen Caseine der Muttermilch. Sie bilden einen wichtigen und essentiellen Bestandteil der Muttermilch und besitzen nicht nur nutritive sondern auch immunmodulatorische Eigenschaften. In dieser Arbeit wurde der mögliche Einfluss von αS1-Casein (CSN1S1)-IgG Antikörpern (AK) auf die Entwicklung von entzündlich rheumatischen Systemerkrankungen untersucht. Die IgG-Antikörperkonzentration gegen CSN1S1 in Seren von 130 Probanden mit rheumatischen Erkrankungen und 86 gesunden Probanden wurde verglichen. Die Messung erfolgte mit einem speziell modifizierten Auto-Display Enzyme-Linked Immunosorbent Assay (ELISA). Als mögliche Einflussfaktoren auf die Antikörperkonzentration wurden der Stillstatus, das eigene praktizierte Stillverhalten, das Geschlecht, die Erkrankung sowie die Raucheranamnese evaluiert. Für die statistischen Analysen wurden gemischt-lineare Modelle und Interaktionsanalysen durchgeführt.
Es zeigte sich, dass männliche Teilnehmer, die als Säugling gestillt worden sind, im Vergleich zu gestillten weiblichen Teilnehmerinnen geringere CSN1S1-IgG- Konzentrationen aufweisen. Hingegen wurden keine Assoziationen der CSN1S1-IgG-Konzentration zu den erfassten entzündlich-rheumatischen Erkrankungen oder den anderen Einflussfaktoren detektiert.

The etiopathogenesis of inflammatory rheumatic diseases like rheumatoid arthritis (RA), systemic lupus erythematosus or vasculitides has not been completely elucidated, and is likely multifactorial. One hypothesis consists in a flawed immune system development during infancy. Caseins are amongst the earliest proteins and thereby potential antigens administered to the newborn in the course of nursing. They are an essential component of breast milk and possess nutritional and immunomodulatory properties. In this thesis, the potential role of αS1-Casein (CSN1S1)-IgG antibodies on the development of inflammatory rheumatic diseases was examined.
The IgG concentration of antibodies against CSN1S1 in serums of 130 subjects with rheumatic diseases and 86 healthy controls was compared. The measurement was performed with a specially modified Auto-Display Enzyme-Linked Immunosorbent Assay (ELISA). As potential influencing factors breastfeeding status, own practised breastfeeding, gender, rheumatic disease status, and smoking habit were evaluated. For the statistical analysis, mixed-linear models and interaction analyses were performed.
It was demonstrated that male participants, who were breastfed as an infant, showed lower CSN1S1-IgG-concentration in comparison to female participants. This result remained significant after being adjusted for the other influencing factors. No further associations, especially to rheumatic diseases, were noted.
