Beschreibung: | Den primären Therapieansatz bei der operativen Versorgung von Aortenklappenvitien stellt die Aortenklappenrekonstruktion dar. Sofern diese nicht möglich ist, gilt der Aortenklappenersatz als Standardverfahren. Hierbei treten jedoch klappenspezifische Komplikationen auf. Diese sind besonders bei mechanischen Aortenklappenprothesen, welche bei jüngeren Patienten indiziert sind, gravierend. Die Ross-Operation bietet für diese Patienten eine geeignete Alternative. Allerdings gibt es mehrere Abwandlungen der Operationstechnik, die dementsprechend auch das Langzeitergebnis beeinflussen. In unserer Klinik wurde die Ross-Operation ab 2009 als „full root mit reinforcement“, also als kompletter Wurzelersatz mit Verstärkung, durchgeführt.
Es wurden insgesamt 100 Patienten nachuntersucht, die im Zeitraum 08/2008 bis 02/2014 eine isolierte Ross-Operation (Ross-Gruppe) oder einen mechanischen Klappenersatz erhalten haben (mAKE-Gruppe). Unter Berücksichtigung der Strukturgleichheit ergab sich aus einer „matched pair“-Analyse eine endgültige Stichprobe von 80 Patienten. Diese unterteilt sich in jeweils 40 Patienten für jede Gruppe. Die Nachuntersuchung erfolgte Ende 2014, wobei der Schwerpunkt, auf der echokardiografischen 2-Jahres-Nachkontrolle, dem postoperativen Verlauf sowie dem aktuellen Gesundheitszustand liegt. Die zu erhebenden Parameter umfassen echokardiografische Ergebnisse, unter Berücksichtigung der Druckgradienten an der Aortenklappe, das peri- und postoperative Ergebnis, insbesondere hinsichtlich postoperativer Komplikationen, sowie die jetzige Lebensqualität der Patienten nach dem EQ-5D-3L Fragebogen. Trotz längerer Operationszeit und intraoperativer Ischämiezeit des Herzens, zeigte die Ross-Gruppe, eine geringere postoperative Beatmungszeit und Gesamtaufenthaltsdauer. Hinsichtlich der perioperativen und frühpostoperativen Komplikationen, der Lebensqualität und des Einflusses der unterschiedlichen Hämodynamik auf die Herzfunktion, fanden sich keine statistisch signifikanten Unterschiede zwischen den beiden Gruppen. Die gemessenen Druckgradienten waren in der Ross-Gruppe signifikant niedriger und mit den Ergebnissen anderer Arbeitsgruppen vergleichbar. Die Gesundheitsempfindung der Patienten nach Ross-Operation war signifikant höher. Die Risikofaktorenanalyse ergab ein Risiko für Verziehungen des Autografts bei vorangegangener Operation an der Aortenklappe sowie bei akuter Aortenklappenendokarditis. Die Inzidenz schwerer thromboembolischer Komplikationen sowie die Mortalität waren zum Zeitpunkt der Nachuntersuchung in der Patientengruppe nach mechanischen Klappenersatz signifikant höher. Die vorliegende Studie konnte nachweisen, dass die Ross-Operation als eine vom Gesamtrisiko her vergleichbare Alternative zum mechanischen Klappenersatz zu werten ist, die bei symptomatischen Patienten, die nicht voroperiert sind und keine Endokarditis haben, in Erwägung gezogen werden sollte. | |||||||
