Beschreibungen: | Zwischen Februar 1991 und Januar 1995 fuehrten wir eine prospektiv
randomisierte Studie zum Einsatz von Filgrastim (Granulocyte
Colony-Stimulating Factor, G-CSF) nach allogener
Knochenmarktransplantation (KMT) von HLA-identischen Geschwisterspendern
durch. 22 Patienten erhielten G-CSF in einer Dosis von 10 µg / kg
Koerpergewicht ab dem ersten Tag nach der Transplantation (Gruppe
KMT+G-CSF). 23 Patienten wurden dem Kontrollarm (Gruppe KMT) zugeteilt.
Zwischen Januar 1995 und Mai 1997 erhielten alle weiteren Patienten eine
allogene periphere Blutstammzelltransplantation (PBSZ-T) ihrer
HLA-identischen Geschwisterspender (Gruppe PBSZ-T, n= 29). Ziel dieser
Arbeit war es, den Einfluss von G-CSF nach allogener KMT und der
Verwendung von peripheren Blutstammzellen (PBSZ) anstelle von
Knochenmarkstammzellen (KMSZ) auf die haematopoetische Regeneration und
den klinischen Verlauf nach allogener Stammzelltransplantation (SZT) zu
untersuchen. Die mediane Nachbeobachtungszeit betrug fuer die Patienten
der Gruppe K
Hinweise auf klinisch relevante Vorteile durch die Verabreichung von
G-CSF nach KMT sahen wir nicht. Wir konnten keinen Unterschied in der
Regeneration der Erytho- oder Megakaryopoese, im Bedarf an Antibiotika,
in der Zahl der Fiebertage, in der Inzidenz und Auspraegung von schweren
Infektionen sowie der akuten oder chronischen Graft-versus-Host Disease
(GvHD), in der Dauer der stationaeren Therapie, in der Mortalitaet oder
in der Rezidivwahrscheinlichkeit feststellen. Patienten, die PBSZ
erhielten, benoetigten im Vergleich zu den Patienten, die KMSZ erhielten,
signifikant weniger Thrombozytenkonzentrate (38 vs 68 Einheiten, p =
0,028), hatten eine signifikant geringere Inzidenz von schweren
Infektionen (Grad 3 und 4 nach WHO: 7 % vs 30 %, p = 0,028), signifikant
seltener Erreger in den Blutkulturen (0 % vs. 52 %, p < 0,001) und
wiesen tendenziell seltener pulmonale Infiltrate (4 % vs 17 %, p = ns)
auf. Sie erlitten jedoch signifikant haeufiger eine chronische GvHD vom
Grad extensive (36 vs 16 %, p = 0,007). Es ergab sich kein
signifikanter Unterschied im Wiederanstieg der Thrombozytenzahlen, des
Haemoglobinwertes, der Anzahl der Fiebertage, der Dauer der intravenoesen
Antibiotikatherapie und des stationaeren Aufenthaltes, der Inzidenz oder
Auspraegung akuter GvHDs, der UEberlebens- und Rezidivwahrscheinlichkeit
gegenueber den Patienten, die KMSZ erhielten. In Stammzellassays, die mit
am Tag 14 nach Transplantation entnommenen Knochenmarkzellen angefertigt
wurden, w Hoehere CD34+Zellzahlen zur PBSZ-T gingen mit einer kuerzeren
Agranulozytose und kuerzeren Zeiten der GvHD-Prophylaxe einher, waehrend
eine hoehere Anzahl kernhaltiger Zellen die Agranulozytosedauer nicht
verkuerzten, jedoch eine laengere GvHD-Prophylaxe notwendig machten.
Schlussfolgerungen: Das Verfahren der PBSZ-T verdraengt auch bei
allogener SZT die klassische KMT zunehmend. Es kann nicht ausgeschlossen
werden, dass die Inzidenz der chronischen GvHD nach PBSZ-T zunimmt.
Ansonsten ist nach unseren Untersuchungen die PBSZ-T in allen
wesentlichen klinischen Verlaufsparametern mit der KMT vergleichbar oder
ihr sogar ueberlegen. Kuenftige Studien sollten sich auf Verbesserungen
der Verfahren zur Mobilisierung und Selektion von Stammzellen zur PBSZ-T,
Verbesserungen in der GvHD-Prophylaxe und der sonstigen supportiven
Therapie sowie die Langzeitbeobachtung der Patienten und Spender
konzentrieren. Die Rolle der Stammzellkultivierung zur UEberwachung der
haematopoetischen Regeneration und als Prognosefaktor fuer den klinischen
Verlauf nach Transplantation sollte in kontrollierten Studien untersucht
werden. We performed a prospective, randomised trial to investigate the role of Filgrastim (granulocyte-colony-stimulating-factor, G-CSF) after allogeneic bone marrow transplantation from HLA-identical siblings. 22 Patients received G-CSF in a dose of 10 µg / kg bodyweight from the first day after transplantation (group BMT+G-CSF). 23 Patients were randomised to the control group (group BMT). After the end of this prospective trial, another group of 29 patients received a transplantation with peripheral blood stem cells (group PBSC-T). The aim of this study and a review of the relevant literature was to investigate the influence of G-CSF-therapy after bone marrow transplantation and the infusion of peripheral stem-cells instead of bone marrow cells on the hematopietic recovery and the clinical outcome after allogeneic progenitor cell transplantation. The median time of last followup was 47 month for the group BMT, 43 month for the group BMT+G-CSF, and 11 month for the PBSC-T- group. The three groups were we Compared to the control group BMT the median time of agranulocytosis was shortened by 3 days by the use of G-CSF after BMT (17 vs. 20 days, p = 0,053) and one day by the use of PBSC (19 vs. 20, p = 0,042). According to clinical relevant outcome parameters, we could not observe any relevant advantage considering the use of G-CSF after allogeneic BMT. There was no difference in time of regeneration of granulocyto- and magacaryopoiesis, in the need of antibiotics, in the number of days with fever, in the incidence of severe infections, in the incidence of acute and chronic graft versus host diseases (GvHD), in the time of in hospital treatment, in mortality and in prevention of relapse. In comparison to patients receiving BMC patients transplanted with PBSC needed significantly fewer units of thrombocyte concentrates (38 vs. 68 units, p = 0,028), had a significantly lower incidence of severe infections (grade 3 and 4 according to WHO: 7 % vs. 30 %, p = 0,028), had significantly fewer positive blood cultures (0 % vs. 52 %, p < 0,001) and had significantly fewer pulmonary infiltrations (4 % vs 17 %, p = ns). On the other hand the incidence of extensive chronic GvHD was significantly higher in patients receiving PBSC (36 vs 16 %, p = 0,007). There were no significant differences in the recovery of megacaryopoiesis and erythropoiesis, the number of days with fever and antibiotics, duration of in hospital stay, the incidence of acute GvHD, mortality and likelihood of relapse. In stem cell assays performed by bone marrow cells taken from the patients on day 14 after transplantation patients of the PBSC-T-group compared to patients of the BMT-group showed significantly higher numbers of colony-forming-unit-of-granulocytes (CFU-G)-cellclusters after one week of incubation (67 vs 0, p = 0,012) and significantly higher numbers of CFU-G-cellcolonies after 2 weeks (114 vs 0, p = 0,047). Summerizing all patients of the three treatment-groups, patients who showed more than the median number of CFU-G-colonies after two weeks of incubation had a significant lower incidence of severe infections compared to patients with lower numbers of CFU-G-colonies (infections grade III+IV accoding to WHO: 0 vs 39 %, p = 0,008). In PBSC-T higher numbers of CD-34-positive cells in the infused apharesis-products enabled a significantly shorter time of agranulolcytosis and a significantly lower time of GvHD-prophylaxis. On the other hand a higher number of mononuclear cells was associated with a longer Conclusions: PBSC-T is becoming more and more common in the allogeneic progenitor cell transplantation. At the present time it cant be definitely ruled out, that the use of PBSC is connected with a higher incidence of chronic GvHD. According to our investigations and the review of the relevant literature PBSC-T is equal or even superior in all important clinical parameters. Studies performed in the future should focus on improvements in stem cell mobilisation and harvest for PBSC-T, prevention of GvHD, other supportive therapy options and the long-term observation of PBSC-donators. The role of stem cell assays as an additional method of surveillance of hematopoietic recovery and its prognostic value should be investigated in controlled clinical trials. | |||||||
