Beschreibung:Seit der DRG(Diagnosis Related Groups)-Einführung in Deutschland erfolgt die Krankenhaus-Vergütung in der Neonatologie anhand des Geburtsgewichts. Um eine durch dieses neue Finanzierungssystem mögliche Beeinflussung der registrierten Geburtsgewichte zu untersuchen, wurde in dieser retrospektiven Studie die Verteilung der VLBW (Very Low Birth Weight (< 1500 g)) vor und nach DRG-Implementierung 2004 in Deutschland verglichen.
Hierfür wurden 11943 Neugeborene mit Geburtsgewicht < 1500 g, welche in den Jahren 1999-2001 und 2004-2006 in Nordrhein-Westfalen geboren wurden, in die Untersuchung eingeschlossen.
Nach Einführung des DRG-Systems konnte eine signifikante Veränderung der VLBW-Verteilung um die 1500 g- und 1000 g-Grenze nachgewiesen werden.
So wurden 2004-2006 unterhalb der 1500 g-Marke mehr VLBW erfasst, als 1999-2001 (50 g-Klasse (1449-1499 g): p = 0,021; 40 g-Klasse (1459-1499 g): p = 0,0077; 30 g-Klasse (1469-1499 g): p = 0,0044; 20 g-Klasse (1479-1499 g): p = 0,0011; 10 g-Klasse (1489-1499 g): p = 0,0055).
Oberhalb der 1000 g-Gewichtsgrenze konnte eine signifikante Abnahme der VLBW post-DRG jedoch nur in der 40 g-Klassierung (1000-1040 g) nachgewiesen werden (p = 0,0484). In den übrigen Klassierungen war die Veränderung der Verteilung nicht signifikant.
